Carefully review the acceptable forms of residency verification below and upload one complete, unaltered item that satisfies the requirement. The image must show the full page with only sensitive information such as SSNs or account numbers redacted. Incomplete or invalid documents (partial, edited, or an item not listed below) will be rejected.
Acceptable Forms of Proof of Residency:

Mortgage statement (dated within 60 days)

Closing documents on a recent home purchase (dated within 90 days)

Signed and current lease agreement

Current utility bill (e.g., electric, gas, water, internet, cable; dated within 60 days)

Bank statement (dated within 60 days)

Voter registration card

Form IL-1040, US-1040, or other tax return form from the most recent tax year

An official piece of mail (submit the full letter, not a picture of the envelope) from a federal, state, or local government agency (dated within 60 days)
